Release of Liability, Waiver of all Possible Claims and Assumption of Risk

I hereby acknowledge that I have agreed to meet with Melanie Warburton, meTIME massage therapy (the “MTMT”), at Salterhebble hill (the “Facility”), for the purpose of receiving hands on Massage therapy treatments and teaching (the “Services”).
I acknowledge and accept that there is a risk that I could be exposed to COVID-19 while attending at Salterhebble hill. I also acknowledge and accept that while receiving services, the MTMT may need to be closer than the recommended social distancing guidelines to assess and/or treat me. I acknowledge and confirm that I am willing to accept this risk as a condition of attending at the Facility to receive services from the MTMT.
In consideration of the MTMT agreeing to see me in person at the Facility, I agree to release the MTMT and the Facility, their officers, directors, employees, agents and volunteers (the “Releases”) from any and all causes of action, claims, demands, requests, damages or any recourse whatsoever in respect of any personal injuries or other damages which may occur or arise as a result of exposure to COVID-19 during my visit to the Facility and/or through the provision of services to me by the MTMT.
I do hereby acknowledge and agree that notwithstanding the generality of the foregoing, I declare that I will not commence litigation or otherwise seek to recover damages or other compensation against the Releases based on any action, claim, demand, request, loss or any recourse whatsoever arising from any potential or actual exposure to COVID-19 while attending at the Facility and/or through the provision of services to me by the MTMT. I further acknowledge that the Releases can rely on this Release of Liability, Waiver of all Possible Claims and Assumption of Risk as a complete defence to any and all claims, damages, causes of action, or recourse or liability that may arise at any time.
I have carefully reviewed this Release of Liability, Waiver of all Possible Claims and Assumption of Risk and acknowledge that I fully understand the terms as set out above. I acknowledge that I am signing this Release of Liability, Waiver of all Possibilities .
INFORMED CONSENT FOR IN-PERSON SERVICES DURING COVID-19 PANDEMIC
This document contains important information about our decision (yours and mine) to have massage therapy treatments considering the COVID-19 pandemic 2020/21/22.
Please read this carefully and let me know if you have any questions. When you sign this document, it will be an official agreement between us.
Decision to Meet Face-to-Face and undertake your massage therapy appointment.
We have agreed to meet in person for therapy. If there is a resurgence of the pandemic or if other health concerns arise, however, I may require cancelling your appointment. If you have concerns, we will talk about it first and try to address any issues. You understand that, if I believe it is necessary, I may determine that we re-arrange your appointment booked with meTIME massage therapy for everyone’s well-being.
If you decide at any time that you would feel safer waiting for your appointments or you are showing symptoms, I will respect that decision, if it is feasible and clinically appropriate.
Treatments being offered to you during COVID-19 pandemic.
During this uncertain time, I have taken certain precautions to keep you and me safe .With the variety of treatments on offer its important to wear the necessary PPA, as the Therapy is close contact and can be transmitted face to face following FHT, IFA and GOV.COM GUIDELINES.
Your Responsibility to Minimize Your Exposure
To obtain services in person, you agree to take certain precautions which will help keep everyone (you, me, and our families, [my other staff] and other clients) safer from exposure, sickness and possible death. If you do not adhere to these safeguards, it may result in termination of your therapy.
· You will only keep your in-person appointment if you are symptom free.
· If you have symptoms of the coronavirus, you agree to cancel the appointment ASAP. If you wish to cancel for this reason, I will charge you our normal cancellation fee.
· You will wait in your car or outside until no earlier than 5 minutes before our appointment time.
· You will wear a face mask within the treatment room and face to face consultations.
· You will use alcohol-based hand sanitizer when you enter and leave the therapy room.
· You will bring a bottle of water with you for your aftercare.
· You will have a quick temperature check upon arrival for test and trace purposes.
· You will use the WC toilet facilities before you arrive for your appointment to reduce spread of the coronavirus and infection control.
· You will adhere to the safe distancing precautions we have set up in the therapy room and the main building.
My Commitment to Minimize Exposure
My practice has taken steps to reduce the risk of spreading the coronavirus within the therapy room to keep you and I safe and well.
· I will be wearing PPE face masks, visor, and aprons appropriate to treatments.
· I will have regular covid lateral flow tests and PCR tests when needed
· I will sanitize my hands, wrists forearms and elbows before and after each client.
· Office seating and equipment used in the therapy room has been arranged for appropriate physical distancing.
· The clients seating in the therapy room is disinfected after each client.
· Hand sanitizer that contains at least 60% alcohol is available in the therapy room.
· I will schedule appointments at specific intervals to minimize the number of people in the waiting room and cleaning time.
· I ask all clients to wait in their cars or outside until no earlier than 5 minutes before their appointment times. Do not arrive earlier than needed.
· Credit card, pads, pens, and other areas that are commonly touched are thoroughly sanitized after each use.
· I will ensure the therapy room is well ventilated through the day.
· Tissues and rubbish bins are easily accessed. Rubbish is disposed of on a frequent basis.
· Common areas are thoroughly disinfected at the end of each day.
· The Therapy couch is disinfected before and after each treatment.
If You or I Are Sick
You understand that I am committed to keeping you, me, and all our families safe from the spread of this virus.
If you show up for an appointment and I believe that you have a fever (temperature check) or other symptoms /coughing /colds, or believe you have been exposed, I will have to require you to leave the therapy room immediately. We can follow up with any follow up appointments through telephone communications as appropriate.
If I test positive for the coronavirus, I will be shielding according to Gov.com and notify test and trace, I will notify you so that you can take appropriate precautions.

Your deposit is to secure your booking and is non refundable , if your unable to attend your appointment you must let your therapist know within 48 hours , so we can fill the appointment .
You will be sent a link to pay - late cancellation fee for no shows and last minute cancellations.
Please note - Your deposit is non transferable and non refundable .
